CareScout's Care Coordination Service is an integrated risk management solution that combines the unprecedented results of a best-in-class Care Advocacy and Discount Program with a highly specialized, individually trained, national network of registered nurses called the FieldScout® Network.
FieldScouts complete objective assessments on long-term care claimants and produce high quality, accurate summaries providing the information needed to make timely eligibility determinations. Care Coordination services include:
- Plan of Care Development: By combining specialized nursing resources with proprietary information, CareScout develops a care plan that truly meets the needs of claimants.
- Ongoing Care Coordination: On a regular basis, FieldScouts review the plan of care and modify it as needed to ensure that the claimant continues to receive appropriate services. As needs change, CareScout can provide access to information on about 100,000 long-term care providers and CareScout Ratings® and Reports.
