Through relationships with corporations and families, CareScout is responsible for arranging on-site visits by eldercare professionals to help families assess their needs and develop plans of care. To provide national coverage, CareScout developed its FieldScout® Network (established in 2000). These professionals further promote the mission of helping Americans make intelligent eldercare decisions. If you are a nurse, social worker, or professional geriatric care manager, you may qualify for enrollment in the FieldScout Network.
Members of the FieldScout Network perform many services to:
- Understand and evaluate an individual's physical and cognitive status
- Assess an individual's level of ADL and IADL dependency
- Assess and evaluate caregivers' abilities to perform required care
- Develop a comprehensive plan of care in cooperation with a family and a CareScout Care Advocate
- Evaluate home safety and make recommendations that can positively impact the family's life and ability to age in place
- Identify local providers and/or community resources that meet a family's needs
- Perform a facility evaluation by completing a questionnaire with the facility administrator and collect required documents
