Schedule a care evaluation

Submit our online form to share your details and convenient dates to meet with a nurse from our Field Team.

Meet with your licensed nurse

During your virtual visit, your nurse will ask questions to evaluate physical and cognitive abilities. They’ll also discuss personal activities, goals, and preferences.

Receive your Care Plan

You’ll receive a personalized Care Plan that outlines care, equipment, and resource options to help you get the care and support you need to live the life you want, safely.

Get your plan for confident care decisions

$249 one-time fee

Your Care Plan is a detailed report that sets you up for success by outlining in clear language the supports and tools that can help you achieve a dignified and fulfilling aging journey.

  • Unbiased analysis of physical and cognitive function, to understand strengths and areas for support.

  • Detailed personalized report that addresses the full range of support that can help improve safety and quality of life.

  • Clear recommendations on next steps to consider, including specific community services, care providers, and equipment options.

Maintaining an active social life is as important in older age as it is when we are younger. Community options like assisted living or adult day care can help older adults find friendships and meaningful activity.
