Our first engagement with each client consists of a living room visit aimed at genuinely getting to know the client and his/her family. During this visit, one of our registered nurses conducts a thorough medical assessment which provides our team a comprehensive picture of the client’s medical needs. Additionally, key details about the client (background, likes, dislikes, hobbies, goals, daily schedule, etc.) are secured to properly match the client with compatible caregivers and/or nurses. Depending on the type of care required, reassessments are conducted at 30-, 60-, and 90-day intervals to ensure that care remains on point, and weekly care notes are reviewed by the clinical directors on a weekly basis. Lastly, we truly understand that PCC requires intensive collaboration from all members of an individual’s care team. Consequently, throughout the journey of caring for a client, we work closely and collaboratively with all members of the care team (doctors, therapist, other practitioners, etc.) to ensure that there is purposeful, open communication aimed at supporting the client’s well-being."
