What person-centered care means to us...

At BrightStar Care of Wellington and Palm Beach, we believe that fully understanding each client’s unique needs is essential to providing meaningful care that is on point. Our first engagement with each client consists of a living room visit aimed at genuinely getting to know the client and his/her family. During this visit, one of our registered nurses conducts a thorough medical assessment which provides our team a comprehensive picture of the client’s medical needs. Additionally, key details about the client (background, likes, dislikes, hobbies, goals, daily schedule, etc.) are secured to properly match the client with compatible caregivers and/or nurses. Depending on the type of care required, reassessments are conducted at 30-, 60-, and 90-day intervals to ensure that care remains on point, and weekly care notes are reviewed by the clinical directors on a weekly basis. Lastly, we truly understand that PCC requires intensive collaboration from all members of an individual’s care team. Consequently, throughout the journey of caring for a client, we work closely and collaboratively with all members of the care team (doctors, therapist, other practitioners, etc.) to ensure that there is purposeful, open communication aimed at supporting the client’s well-being.

What would you want to know for family member care ...

I want to know if the agency I am considering utilizes W2 employees or contractors. All of our caregivers are direct W2 employees. This equates to proper training and supervision and eliminates all liability from you as a client. Additionally, I would want to know if care is individualized and governed by a customized care plan and whether or not the agency is committed to maintaining caregiver consistency and continuity of care. At BrightStar Care of Wellington and Palm Beach, we craft a customized care plan for each client that serves as a blueprint on how best to support our clients. Additionally, we work diligently to identify the perfect caregiver/s for our clients and always aim to establish a set schedule and caregiver lineup.
