My parents lived at HarborChase of Dr. Phillips Assisted Living for 41 days (August 15–September 25, 2025). During that time, there were three different Executive Directors (DJ, Tammy, and Heather). The Head of Maintenance (Luis) also left during that period. After I filed a complaint regarding care concerns, the Florida Agency for Health Care Administration (AHCA) conducted an investigation and issued a 48-page Statement of Deficiencies. The findings included failures related to physician order follow-through and medication-related documentation. These records are publicly available through the state. When I moved my parents out, I confirmed the account reflected a zero balance. Our sales representative had indicated we would receive one free month. After move-out, corporate leadership — Katherine (Regional Director of Operations) and Mark (Chief Operating Officer) — provided written assurances regarding credits and refunds. They billed $39,000 for 41 days.
