"We’ve experienced repeated billing increases at FountainView without meaningful changes in care or proper documentation. Despite ongoing efforts to resolve things respectfully, the billing department has been inflexible—adding late fees and trying to withdraw funds beyond authorized limits, even during an active dispute. Most upsetting, staff involved our elderly loved one in financial matters despite clear family delegation, causing unnecessary confusion and stress. If you're considering FountainView, insist on all assessments in writing, review every invoice closely, and document every conversation. Or better yet—keep looking." - Vee… Read more

"My husband, Dennis Wyatt, was a resident for 6 months and his care was amazing. Rose adopted him from day one and knew how to give him his meds in frozen Ensure and was able to feed him all his food. No other facility could do this. She had help from Olga, Darlene, Lou and Mamie. There were others I probably don't mention but just know the care was fabulous compared to Corporate owned facilities. I would highly recommend this facility to anyone who's loved one has Demenntia. Seema and Kim are the best! Love those women." - Karen… Read more

"I had a loved one there recently and Rustic Retreat has got to be the worst nursing home in all of south Florida. They have no call buttons by the residents bed and none in the bathroom should a resident need to call for help. Instead, the owner expects everyone to own a cell phone so they can call the main number at the front desk for help and good luck with that because half of the time nobody answers the phone and you have to leave a message, wait, and hope someone gets to you. The owner looks for the cheapest way out of everything and the food is terrible they serve such small child portions of food and half of the time the food isn't even that good and if you don't like what is being served they offer you peanut butter and jelly sandwich on white bread, they don't even have the more healthy wheat bread. As far as employees go there are no registered nurses working there and nobody checks in the residents rooms to make sure they are ok. The rooms are so small yet residents are expected to pay quite a bit of money in addition to what Medicare pays. They are so cheap at this place too for example 2 days per week you get egg for breakfast, 1 egg and 1 piece of toast and they put a little butter on the toast for you because they are afraid the residents will use too much butter. Soup most of the time it's so terrible and you don't even know what kind of soup it is, forget about traditional chicken noodle soup you almost never see that. Sunday dinners are usually cottage cheese as the main course with a few pieces of fruit from a can! They use too much sodium in the foods as well having no consideration for those with high blood pressure. If you want a snack you have to walk down to the dinning area to get it even if you can't walk that well because they won't bring nor allow eating in the resident room. I say keep your loved ones as far away from this place as you can! They don't even have any activities for the seniors. Most of the food there isn't very healthy or good at all." - N/A… Read more

Amazing Grace Assisted Living Home II is a licensed residential assisted living facility in West Palm Beach. Not a wing of a larger campus — an actual house, in an actual neighborhood, licensed for six residents. That six-resident cap is the reason families choose us. Our daytime staff-to-resident ratio is 1:3, and the home is staffed overnight as well. In practice it means a caregiver who knows each resident's routine, their preferences, and what a bad day looks like for them specifically — the kind of familiarity that simply doesn't develop at 1:10 with rotating staff. All six Amazing Grace homes are owned and directed by Debbie Lytle, RN, who has practiced nursing in Palm Beach County for over 30 years and personally oversees every resident's care plan. When you call, you speak with her. Because of that clinical oversight, we're often able to accept residents with dementia, complex medication regimens, or mobility needs that larger facilities decline. Rates are all-inclusive — no point system, no bill that escalates as care needs increase.… Read more
