"There is no rating below a zero for this facility. The place is filthy & smells. Patient rooms have dirty sticky floors. Bathrooms have mousetraps placed on the floor in them. Patient bedside tables are sticky & coated with spills/leftover food. Patients are not given baths, shaved, hair washed/trimmed, or nails clipped regularly. Bedridden patients are not turned to prevent bedsores. Food served to patients is cold & mostly inedible. I do not know how you pass State inspection.
