"My mother has been at Vitality for a few months. The facility it self is very nice, and there are some GREAT people there. However, they are severely under staffed. They advertise that they clean rooms and do laundry, but in my experience, this sometimes gets overlooked, and I end up doing it for my mom. The bigger issue is the response time when residents need help. My mom has fallen, and no one came when she pushed her alert button. She had to get help from another resident. I have heard from residents that a woman fell out of her bed recently, and was on the floor for a couple of hours. This is unacceptable for an assisted living facility. "Since Alto took over in Oct 2024, this facility has gone downhill—especially in Memory Care. It's now severely understaffed, poorly managed, and unsafe. Temp workers unfamiliar with residents are common, care plans are ignored, and basic needs like toileting and meds are often missed. There were 7 months with no activities. Food is cold or inedible. Several residents have gotten out unsupervised; others were injured. Families who raise concerns face retaliation—some received 30-day notices. Alto does NOT support aging in place. Cameras are allowed but staff are told to cover them, limiting oversight. Communication from management is nearly nonexistent. "Magnolia is not a place I would not want to put my loved one. The management is the worst I have ever seen, uncommunicative ,unprofessional and honorably and book keeping. Staff, when you can find them was Ok. The rooms are Ok but too far from the staff station. Again staff was never around. When my loved one passed, part of the contract we signed, which by way ended up having various errors. But the contract stated that would receive the unused funds for the month that my loved one was not in the facility. My loved one was of little means and we needed those funds to bury our loved one. I would not recommend Magnolia. We have had experience with 4 other care facilities and Magnolia was the worst by far. The only reason we had to place our loved one in Magnolia was because we were between a rock and a hard place for was asked to move our loved one because was mostly bed bound. "I would not recommend sending your loved one to The Oaks at Conyers unless you don’t care about them. Leadership and management are unresponsive, retaliatory, and only want residents with uninvolved families. When we raised valid concerns about poor care, neglect, and unsafe conditions, they discharged our mother instead of addressing the problems. During her two-year stay, we experienced constant understaffing, unclean rooms, residents left soiled for hours, no activities or nursing staff for months, and a filthy facility that often smelled of urine. Staff were lazy, unprofessional, and unsupervised, even falsifying medication records without consequence. Corporate leaders ignored us for over a year. While a few staff members like Melissa, Pauline, and Jennifer were kind, the overall culture is toxic and uncaring.
